Caregiver Assurance™ is a membership program that provides consultative support over the phone and online to aid you in your caregiving journey. We help you maintain your own well-being as you support others in living a fulfilling, independent life.
Caregiver Assurance Members receive:
A dedicated Caregiver Advisor - Receive ongoing support and access to expert advisory services. They are trained in aging services and work to understand your unique situation, developing a program that suits your schedule. Your advisor is a consistent resource, even as your loved one’s needs change over time.
Important resources and referrals to support your loved one - Get expert guidance and referrals to care resources and services throughout the Fairview Health system as well as other services throughout Minnesota.
A personalized online account - Your online account is accessible anytime, anywhere. This allows you to share, store and retrieve educational materials, important documents and service resource information provided by your Caregiver Advisor.
Communication facilitation - If helpful, we can conduct a family care conference to facilitate communication and care planning.
